請在付款和預約確認之前同意以下條款。
1. 我正在參加 GattoQue Healing Centre / NPO 日本靈氣協會香港分會提供的靈氣課程、健康計劃、工作坊和/或其他健康、身體工作、治療、鍛煉和治療藝術活動（統稱“活動”）。
GattoQue Healing Centre/ NPO 日本靈氣協會香港分會（「中心」）及/或其擁有人、經理、教師、工作坊主持人、員工及獨立承包商。
2. 我體認到我必須保持足夠的身心健康才能參加活動。我了解活動可能需要劇烈的體力消耗，我聲明並保證我的身體狀況足以參加活動，並且我沒有任何健康狀況會妨礙我充分參與活動。我認識到活動可能會導致或加重身體傷害或健康狀況。我了解，我有責任在參加活動之前諮詢醫生。如果我這樣做了，我就聽取了醫生的建議。我了解工作室保留以醫療、健身或任何其他理由拒絕我參加任何活動的權利。
3. 我知道參加活動可能會導致高血壓、昏厥、心跳紊亂、身體受傷、心臟病或中風，並可能加劇原有的身體狀況。我了解參加活動可能會導致我的肌肉、背部、頸部和其他部位受傷。我了解我的身體限制，並且我有足夠的自我意識，在受傷或加重現有的傷害之前停止或改變我參與任何活動。
4. 考慮到被允許參加活動，我同意對因參加中心活動而可能產生的已知或未知的任何風險、傷害或損害承擔全部責任，包括因中心或任何免責方（定義見下文）的疏忽而可能造成的風險、傷害或損害。
5. 進一步考慮獲準參加活動，我知情、自願並明確放棄我可能因參與工作室活動而對工作室、其所有者、經理、教師、指導員、研討會主持人、僱員、獨立承包商和工作人員（均稱為“免責方”）提出的任何“索賠”（定義見下文），即使該索賠是因任何免責方或其他任何人的疏忽而產生的。
我同意賠償各免責方，使其免受因對我或任何代表我提出索賠的人提出的任何索賠進行辯護而產生的任何損失、費用或責任，即使該索賠據稱是由任何免責方或任何其他人的疏忽造成的。 「索賠」包括但不限於我、我的配偶、子女或未出生的孩子可能因參與任何活動而遭受的任何及所有責任、索賠、要求、開支、費用、法律訴訟、損害、人身傷害、精神痛苦和痛苦或死亡的訴訟權利（包括任何法律費用或開支）。
6. 我、我的繼承人或法定代表人永久免除、放棄、免除責任，並承諾不因被免除方的任何疏忽或其他行為引起的任何索賠而起訴任何被免除方。
7. 我特此理解，中心可能會不時拍攝、錄影或以其他方式記錄在其工作室發生的課程或活動，並將此類照片和影片放置在其網站上。我特此同意使用可能出現在任何此類照片或影片中的我的圖像。
8. 本協議應根據中華人民共和國香港特別行政區法律解釋並受其管轄，與本協議有關的所有訴訟、訴訟、索賠和程序均應向位於香港有管轄權的法院提起。如果本協議的任何條款被視為無效、非法或不可執行，則不會影響本協議的任何其他條款，且本協議應解釋為如同本協議中從未包含此類條款一樣。
我承認我已仔細閱讀本協議並完全理解其內容。我自願且知情地同意此處所述的條款和條件。我知道，透過簽署本協議，我將放棄實質權利，包括我的起訴權以及我的繼承人、近親、執行人、管理人和受讓人可能對任何被豁免方擁有的某些合法權利。
為了確保您的預訂，我們會要求您在課程/課程之前支付費用。您支付的費用將不予退還。
- 取消政策前 24 小時 - 我們懇請您至少提前 24 小時向我們提供重新安排預約的通知。
如24小時內未有取消而課堂日或療程日沒出現，則視為放棄，費用會自動扣除。或將會收取額外行政費作為重新預約安排。

Terms & Condition Please read before you book
Please agree to following before preceding your payment and booking confirmation.
1. I am participating in Reiki classes, health programs, workshops and/or other wellness, body work, therapy, exercise and healing arts activities (collectively, the “Activities”) offered by GattoQue Healing Centre / NPO Japan Reiki Association Hong Kong Chapter.
GattoQue Healing centre/ NPO Japan Reiki Association Hong Kong Chapter, (the “Centre”) and/or its owners, managers, teachers, workshop presenters, employees and independent contractors.
2. I recognize that I must be in adequate physical and mental health to participate in the Activities. I understand that the activities may require intense physical exertion, and I represent and warrant that I am physically fit enough to participate, and I have no medical condition which would prevent my full participation in the Activities. I recognise that the activities may cause, or aggravate a physical injury or medical condition. I understand that it is my responsibility to consult with a physician before my participation in the activities. If I have done so, I have taken the physician’s advice. I understand that the Studio reserves the right to refuse my participation in any activity on medical, fitness or any other grounds.
3. I am aware that my participation in the activities could result in high blood pressure, fainting, heartbeat disorders, physical injury, heart attack or stroke and may aggravate pre-existing physical condition. I understand that I could experience muscle, back, neck and other injuries as a result of my participation in the activities. I understand my physical limitations and I am sufficiently self- aware to stop or modify my participation in any activity before I become injured or aggravate a pre-existing injury.
4. In consideration of being permitted to participate in the activities, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the activities at the centre, including those which may result from the negligence of the centre or any Released Party (as here in after defined).
5. In further consideration of being permitted to participate in the activities, I knowingly, voluntarily and expressly waive any “Claim” (as defined below) I may have against the Studio, its owners, managers, teachers, instructors, workshop presenters, employees, independent contractors and staff (each, a “Released Party”) that I may sustain as a result of participating in the activities at the Studio even if the claim arises from the negligence of any released party or anyone else.
I agree to indemnify and hold harmless each released party from any loss, cost, or liability incurred in defending any claim made by me or anyone making a claim on my behalf, even if the claim is alleged to or did result from the negligence of any released party or anyone else. “Claim” includes but is not limited to any and all liabilities, claims, demands, expenses, fees, legal actions, rights of actions for damages, personal injury, mental suffering and distress, or death that I may suffer, my spouse, children or unborn child may suffer (including any legal fees or expenses) in connection with participation in any activity.
6. I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue any released party for any claim caused by any negligence or other acts of a released party.
7. I hereby understand that the Centre from time to time may photograph, video, or otherwise record classes or events occurring at its studios and place such photographs and videos on its Website. I hereby consent to the use of my image that may appear in any such photograph or video.
8. This agreement shall be construed in accordance with, and governed by, the laws of Hong Kong Special Administrative Region of the People’s Republic of China and that all actions, suits, claims and proceedings relating to this agreement shall be brought in a court of competent jurisdiction located in Hong Kong. In case any provision of this agreement shall be held invalid, illegal or unenforceable, it shall not affect any other provision of this agreement and this agreement shall be construed as if such provision had never been contained herein.
I acknowledge that I have carefully read this agreement and fully understand its contents. I voluntarily and knowingly agree to the terms and conditions stated herein. I am aware that by signing this agreement, I am giving up substantial rights, including my right to sue and certain legal rights my heirs, next of kin, executors, administrators and assigns may have against any released party.
To secure your booking, we will ask you pay the fee before your class / session. The fee you paid is non-refundable.
– 24 hours before cancellation policy –
We kindly ask you to provide us with at least a 24-hour notice for rescheduling the appointments.
No shows or cancellation without 24hours notice, the prepayment will be deducted automatically. Or the management fee will be charged for the reschedule without 24hours notice.
